Certain features of this Sweepstakes may be different in Canada than in the United States. You are eligible to win a prize if you receive the promotional letter/notice enclosed in this envelope and return it with your Prize Claim/Order Form (Order Form) in accordance with these Official Sweepstakes Rules.
To participate in the Sweepstakes complete your merchandise order on the enclosed Order Form and mail the Order Form using the convenient reply envelope, if supplied. Simply send in your order and you will be automatically entered into the sweepstakes.
Sweepstakes is open to residents of Canada and the United States who have reached the age of 18 by February 1, 2012, except for employees and immediate family members of Clearpoint International Direct Marketing (Sponsor), their agents, affiliates, subsidiaries, retailers, wholesalers, licensees, printers, and advertising and promotional agencies.
All prizes will be awarded via random drawing on or about January 18th, 2013, from all valid entries received by December 31st, 2012 at the addresses listed below.
The Sweepstakes consists of a series of separate Sweepstakes presentations, which may vary in style and content and in most cases will be accompanied by advertising materials offering various products marketed by the Sponsor. Order Forms will be distributed by mail in Canada and the United States and may be distributed by other means as well. All potential recipients of mailings will have a personal claim/entry number assigned by computer. This number appears on the Order Form in proximity to the recipient's name and address and may also appear elsewhere in the Order Form. In order to win and claim a prize, a recipient must return a completed Order Form as indicated above bearing his or her claim/entry number.
No recipient is guaranteed to win a cash prize. Winning such a prize will depend on whether your claim/entry number is chosen in the random drawing, which is not guaranteed.
The following prize(s) will be awarded to eligible recipients who return a valid Order Form and have been chosen in the random drawing (and, in Canada, who have correctly answered the skill-testing question - see below). One (1) Grand Prize Winner will receive Five Thousand Dollars ($5,000). Total retail value of all prizes is Five Thousand Dollars ($5,000); (value of prizes are in CDN currency). For Canadian residents only, in order to win a prize, a recipient whose claim/entry number has been determined by the Sponsor to be a winning claim/entry number must correctly answer a mathematical skill-testing question administered during a mutually convenient pre-arranged telephone call.
The chances of winning will depend on the number of entries received during the course of the Sweepstakes, which we anticipate to be between 45,000 and 500,000. All such entries will have the same chance to win and the estimated odds of winning a prize in any random drawing will be between 1:45,000, and 1:500,000.
Commencement date for the Sweepstakes is January 1st, 2012. The Sweepstakes deadline for receipt of all entries is December 31st, 2012. Final determination of all winners will be made on or about January 18th, 2013 and all prizes will be delivered within 60 days of the Sweepstakes deadline date.
No purchase necessary to enter or win a prize. To be eligible to win a prize without ordering any advertised products, (a) Complete all sections of the enclosed Order Form; (b) circle your computer-generated claim/entry number which appears immediately above your name and address at the left; and (c) mail the Order Form to the address listed below. Use the convenient reply envelope, if supplied.
The Sweepstakes is being administered by the Sponsor. Decisions of the independent judging organization, who will supervise the awarding of prizes, are final. Sponsor is not responsible for lost, late, misdirected, incomplete, illegible, illegitimately received or tampered with entries. All entries become the property or the Sponsor. Sponsor is not responsible for Sweepstakes claim/entry numbers that are not delivered or are lost in the mailing preparation process. No mechanical reproductions accepted.
Winners will be notified by mail. All prizes must be accepted as awarded and no substitutions apply. Prizes are not transferable prior to awarding. Prize winners will be required to sign an affidavit of eligibility and may be requested to sign a publicity release and return it to the Sponsor within fourteen (14) days of Sponsor's attempt to notify, failing which, prize may be forfeited and awarded to another entrant.
All federal, state, provincial and local laws and regulations apply. Void where prohibited. Taxes are the sole responsibility of the prize winners.
