Official Sweepstakes Rules
To participate, simply fill out your participation/order form.
Please ensure your name, address and telephone number, appear on the participation/order form.
Eligible entries must be received no later than December 24, 2009.
The following prizes will be awarded: One (1) Grand Prize of $5,000,
One (1) First Prize of $1,000, One (1) Second Prize of $500. All prizes are in Canadian Dollars.
For mail-in participation forms, one (1) Bonus Prize of $1,000 will be awarded to the winner of the Grand Prize if
that individual returns the chosen winning order form within 21 days of receiving their order form – the postal stamp
will serve as proof of mail-in date. All participation/order forms must be mailed to Clearpoint P.O. Box 3508, Champlain, New York, 12919.
Total retail value of all prizes, including the Bonus Prize is Seventy Five Hundred Dollars ($7,500). All winners will be notified
by mail. All prizes must be accepted as awarded. To receive a prize, winners will be required to answer a time limited,
mathematical skill-testing question administered by a mutually convenient telephone call. No purchase necessary to enter.
Your entry may be disqualified if the above steps are not followed. For all entries, enter as often as you like.
Commencement date for the sweepstakes is October 1st, 2008. The Sweepstakes deadline for receipt of all entries is
December 24, 2009. Final determination of prizewinners will be made by computerized random drawing from all eligible entries
received, on January 14, 2010 at one o’clock. All prizes will be distributed within 60 days of the determination of prizewinners. Winners will be required to sign an affidavit of eligibility and a publicity release permitting sponsor’s use of their name and photograph and return it to the sponsor within 14 days of sponsor’s attempt to notify, failing which, prize may be forfeited and awarded to another entrant. Void where prohibited by law. Taxes are the sole responsibility of the prizewinner.
Sweepstakes is open to all residents of Canada and the United States who have reached the
age of 18 by January 14, 2010, except for employees of Clearpoint International Direct Marketing Inc. and their agents,
affiliated, subsidiaries, retailers, wholesalers, licensees, printers, advertising and promotional agencies and persons
with whom they are domiciled.
